This PDF is a scan of the official petition, as filed by Rep. Peter Silva with the New Hampshire House in May 2011. In August, this petition touched off a strange incident. My version of the story is as follows:
The petitioner, one Denise-Marie McIntosh, discovered that I had posted the petition, and for whatever reason she was expecting it to say something different. It was a mixup between herself and her sponsor, Rep. Peter Silva. He filed something which was virtually identical to a petition she filed in 2010, which was not what she had in mind. Ms. McIntosh first asked that the petition be amended, and then she and Rep. Silva asked that the petition be withdrawn altogether. The way I read the House Rules, even the committee itself is not allowed to withdraw a petition without acting on it. One problem which resulted from the withdrawal of the petition was that some local officials were (or so I was told) planning to come in and defend themselves before the committee, but were unable to do so.

PETITION FOR REDRESS OF GRIEVANCE
TO: The Honorable House of Representatives

SUBJECT: Grievance of Denise-Marie McIntosh, Nashua, New Hampshire
Your Petitioner, Representative Silva on behalf of Denise-Marie McIntosh of Nashua, hereinafter presents the following summary of her grievance against Judge Thomas Bamberger and the State of New Hampshire, Department of Health and Human Services, Division of Children, Youth, and Families (DCYF) and invokes the constitutional authority and duty of the Honorable House of Representatives pursuant to said Articles 31 and 32 to bring about redress:
Grievance against Judge Bamberger for giving legal advice to William McIntosh to pursue custody in the Superior Court during the course of district court hearings and against DCYF for the refusal by the Nashua visitation center to host and supervise the visitation required by the parenting plan of the Superior Court, effectively denying contact.
Wherefore, your Petitioner prays that the House of Representatives:
Determine if there was maladministration on the part of Judge Bamberger and consider commencing Articles of Impeachment against such Judge.
Introduce legislation to require DCYF to host supervised visitation required by the Superior Court.

There was more info in Ms. McIntosh's 2010 Petition #1: not much more, but a little more. After a longish prolog, the actual grievances were stated very briefly as follows:
During the course of the District Court hearings, Judge Bamberger gave legal advice to William McIntosh to pursue custody in the Superior Court matter. [The petition doesn't specify what "the Superior Court matter" was.]
The parenting plan of the Superior Court instituted by Marital Master Alice Love requires Denise-Marie McIntosh's visitation to be supervised by the Nashua visitation center, which has refused to host the visitation effectively denying contact.
The DCYF does not run the Nashua visitation center. The center is run by a local non-profit agency, and there are several other centers in the general vicinity of Nashua. It was certainly not governmental wrongdoing for a private organization to refuse to be part of the McIntoshes' visitation plan. It wasn't even nongovernmental wrongdoing. But that's just my opinion: Ms McIntosh feels otherwise.
